Condition Management Programmes


Help for Health
– Generic group programme

Holistic Resources developed this programme specifically for people suffering from chronic anxiety, depression or physical conditions who were claiming Incapacity Benefits. The effects of unemployment and long-term illness can lead to psychological disorders and debilitating physical symptoms for which no medical explanation can be found. Incapacity and other benefits, rather than helping the patient, may actually compound the problem and more effective ways of management need to be found.
In 2004 Holistic Resources was contracted by the Teaching Primary Care Trust and Jobcentre Plus to be a partner in the Condition Management Programme (CMP) part of the East Lancashire Pathways to Work pilot scheme. The CMP is a package of health initiatives designed to help and empower people to improve their health, gain control over their illnesses to enable them to progress back towards work, training or other meaningful activities.
‘Help for Health’ is an 8-week programme for groups of up to eight in which patients with diverse medical and psychological conditions can learn to manage their various disorders, support each other and improve their well-being. Therapists are trained to deliver the programme and each client receives a full Handbook and CD ROM to assist them continue the learning at home. Help for Health is an integral part of the CMP, and has been extended across all Lancashire from 2006.

The partnership between the Teaching Primary Care Trust and Holistic Resources has grown to become an excellent model of a public body working with and supporting a social enterprise. The support and encouragement from CMP and the integration of NHS best practice into the programme has ensured that Help for Health has developed into a robust, quality model and current work on the quality systems will enable the programme to meet NHS procurement guidelines.

The programme aims to:
Provide structured group therapy in which patients with anxiety and depression can learn to understand and manage their various problems
Change the way people feel by changing the way they think
Dealing with uncomfortable feelings
Pain Management
Stress Management
Relaxation

The programme includes:  
Cognitive-behaviour therapy The exclusion criteria:
Relaxation Patients with psychotic illness
Guided imagery Patients who actively abuse street drugs or alcohol
Education Patients with a known personality disorder
